Years of Service Awards

One of the many highlights of the annual CCCEOPSA conference is honoring our colleagues through the CCCEOPSA Conference Service Awards and Years of Service Recognition. Complete the form below to be recognized for the Year of Service Award. Service awards are presented to individuals who have dedicated 5, 10, 15, 20, 25, 30, 35, or 40 years of service to the EOPS program. (Please note, one person per submission.)

The deadline to complete the form is Friday, August 14th, 2026, by 5:00 p.m. For questions, contact Evelyn Garcia. Please note that names or nominations will not be accepted for recognition at the CCCEOPSA conference.
